ClubCare is dedicated to providing comprehensive Quality Home-Health and Companion Care to our clients. The goal of ClubCare is to raise the health status of our clients to the highest possible level while preserving each client's dignity.

ClubCare Caregivers assist individuals who wish to remain in their own homes, but who desire help with day-to-day activities. Our ClubCare staff provides care and companionship for those with long-term disabilities, Alzheimer's disease and those recovering from a stroke, heart attack, surgery or other serious illnesses, as well as those who desire assistance with daily activities because of arthritis, diminished eyesight, hearing loss or other normal occurrences of the aging process. ClubCare can provide private duty care in a hospital or a skilled nursing facility.

Paragon Rehabilitation

Paragon Rehabilitation, Inc. is dedicated to partnering with Jacaranda Trace to provide on-site quality care today and into the future, with the attentive and personalized customer service our residents expect. Paragon Rehab provides a comprehensive Rehab Program consisting of Occupational Therapy, Physical Therapy, and Speech/Language Pathology services directed by an on-site Rehabilitation Coordinator. If the need arises for these services, Paragon Rehab's facility is right here at Jacaranda Trace.
