Wellness at Jacaranda Trace The Jacaranda Trace campus is fully equipped to provide seniors with a broad spectrum of specialized healthcare services provided by physicians, nurses, therapists, specialized caregivers and in-home health care through its Club Care program. For residents with assisted-living needs, The Inn at Jacaranda Trace provides individually customized, around-the-clock care in a residential setting. Our Club Care provides a nurse care manager who oversees your health care needs and acts as a liaison to the MD, hospital and other health care providers. In-home assistance with activities of daily living, such as shower assist, light housekeeping, laundry, meal preparation, shopping and transportation. Medication re-ordering and weekly set up by RN Care manager can also be provided. Alliance home Health Services provides in-home physical, speech, occupational therapies (all located within our Clubhouse at Paragon Rehab). Our Wellness program also provides in-home X-rays, and labs. Our Home Health services (Alliance) coordinates the visits of Jacaranda Trace affiliated Podiatrist, Audiologist, Dermatologist and specialized medical services for Parkinson’s Disease. Home Health assists with appointments to the WellnessCenter physician twice a week. There is a dynamic YMCA Lecture Series; on-site YMCA personal trainers; nutrition workshops and health awareness classes such as water aerobics and pool volleyball; cardio workout; body toning and low impact aerobics. The Jacaranda Trace Signature Wellness Program experience provides a positive influence to every aspect of your life. |
